We believe each individual of Empowered Living Services is an extension of our own families. We encourage every individual to lead a lifestyle as active and social as their abilities allow. Supervised and assisted by our experienced 24/7 awake staff, members of our home make meals in the kitchen, gather for games, crafts and experience optimal integration with the greater community.
Our residential group home is designed to meet the specific needs of adults with intellectual disabilities and adults with Autism. This home is beautifully furnished and features a spacious kitchen with dining area, 2 private bedrooms and 1 shared bedroom, 2 full bathrooms, a comfortable gathering area, and large outdoor space.
The purpose of this ID/DD Wavier home is to provide support in the community rather than in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ID).
Targeted Population: Individuals 18+ who are at developmental risk and/or individuals who have Intellectual Disability (ID). All individuals must:
(1) Meet the ICF/IID level of care criteria;
(2) Require waiver services within 30 days; and
(3) Be determined that community-based services under the waiver are the critical services that enable the individual to delay or avoid placement in an ICF/IID or promote exiting from either an ICF/IID or another institutional placement.
Admission Criteria: Program is administered by the Department of Behavioral Health and Developmental Services (DBHDS) and the Department of Medical Assistance Services (DMAS). The individual must be eligible for Medicaid and meet screening criteria.
